The pitch of C140 is off. This is probably because the c140 clock is set to the wrong number.
When I set the c140 clock to 0x7d00, I got the correct pitch.

The clock should be correct for the libvgm-based VGMPlay versions(0.50.0+), the clock value you have is based on older versions of VGMPlay thus it's incorrect. Now, I do have to relog these in the future because the sound update rate is incorrect since Namco System 21 shares the same sound system as the Namco System 2. Once March rolls over, this is the first thing on my list I want to tackle.
